REFERRING DENTIST:
Sign Form and give/mail to “new” Referred Dentist
Referring Dentist must have current account balance
New Dentist must accumulate a minimum of $5,000 of restoration sales within 90 days of receipt of Referral Form
New Dentist must not have had any business with Oral Designs in the previous 90 days
Incentive mailed to Referring Dentist upon requirement fulfillment
REFERRED “NEW” DENTIST:
10% OFF 1st Case (labor only)
Must submit Referral Form with initial case
